SPN 3216 FMI 10: NOx sensor rationality fault – sensor disagreeing with expected values

What this fault means

SPN 3216 is commonly associated with an upstream or intake NOx measurement on modern heavy-duty aftertreatment systems. FMI 10 indicates an abnormal rate of change or a signal that is not behaving as the ECM expects. Exact definitions vary by manufacturer and calibration.

This does not automatically mean the entire SCR system has failed. The ECM may simply be seeing NOx data that changes too quickly, too slowly or inconsistently compared with other operating conditions.

What you may notice

The truck may display an aftertreatment warning, check-engine light or inducement warning. Some faults appear intermittently and may be easier to reproduce after the exhaust system reaches operating temperature during a longer drive.

What to check first

Inspect the NOx sensor wiring, connector and module for heat damage, corrosion or poor connections. Check for exhaust leaks that could affect the sensor's measurement.

Use live data to compare NOx readings under stable engine conditions. Look for a value that freezes, jumps abruptly or behaves very differently from what the engine and aftertreatment system are doing.

Do not replace the sensor blindly

A failing NOx sensor is one possibility, but DEF quality, SCR performance, exhaust leaks, wiring problems and other aftertreatment faults can affect the diagnosis.

Review companion codes before replacing parts. If wiring and exhaust integrity are good and the sensor data remains implausible, follow the OEM test procedure for the NOx sensor.
